Support at home
Care at home that helps young people thrive.
Planned care visits for young people and young adults in their own home or family home. We help with the things that have become difficult, while protecting the routines, friendships and ambitions that matter to them.

An example day
Support that fits around their life.
Visits are planned around school, college, work and family routines.
Ready for the day
Washing, dressing, breakfast and medication, as agreed.
After school or college
A snack, a catch-up and help winding down.
Tea and routines
Preparing a meal together and any evening medication.
Building independence
Practising skills like cooking, money or travel.
On-call
Help is a phone call away, day and night.
Illustrative example. Support is set by each person’s assessment and support plan.
Help with everyday life
Practical help, delivered with dignity.
Depending on the assessment and agreed care plan, visits may include:
- Getting washed, dressed and ready for the day
- Support with meals and drinks
- Medication support, where assessed and included in the plan
- Getting ready for school, college, work or appointments
- Food shopping and agreed household tasks
- Help with daily routines and winding down in the evening
We talk with the young person about what they would like help with, what they prefer to do themselves and how they want staff to support them.
Building independence
Skills learned at their own pace.
Visits are a chance to practise the everyday skills that lead to a more independent life, with support that steps back as confidence grows.
- Cooking simple meals and planning food
- Managing money and budgeting
- Travelling safely and using public transport
- Self-care and keeping on top of routines
- Keeping appointments and managing a calendar
- Speaking up about what they want
Families and short breaks
Support for the whole family.
Parents and carers know their young person best. We involve them in planning, keep them informed and respect the young person’s privacy as they grow older.
Planned visits can also give families regular time to rest, work or spend time with other children, knowing their young person is well supported.
- Involving parents and carers in planning
- Regular updates after visits
- Short breaks for family carers
- Respecting the young person’s privacy
Moving to adulthood
Continuity through big changes.
The move from children’s to adult services can be unsettling. Consistent support from familiar staff helps nothing get lost along the way.
Plan early
We join transition planning with families and professionals.
Keep the team
Familiar staff stay with the young person where possible.
Review together
Support changes as their goals and needs change.
Visits and reviews
Visits planned around their needs.
Before care starts, we assess needs and talk through the proposed visits. We agree the tasks, timing and costs so everyone knows what to expect.
The care plan gives staff clear guidance about preferences and agreed support. Reviews are a chance to say what is going well, raise concerns or discuss changes.
Visit availability depends on location, the support required and staff availability. Contact us to check whether we can support you in your area.

Questions
Support at home FAQs
Answers to the questions we are asked most often.
How long is a visit?
Visit length depends on the tasks in your care plan. We agree the timing and duration of visits with you before care begins.
Will I see the same carers?
We aim to keep a small, consistent team for each person, so you get to know the people who support you.
What if a carer is running late?
The office will let you know. Outside office hours, our on-call team is available on 07379 542527.
Can I change my visits?
Yes. Contact the office and we will look at what is possible. Larger changes may need a review of your care plan.
How is home care paid for?
Care may be funded by the local authority, through a direct payment or personal budget, or paid for privately. We can explain the options when you contact us.
